Duty-Free Concessions Application Form

Applicants can use this form to request permission to import the following items free of duty and taxes including Value Added Tax (VAT) as per directive from the Ministry of Finance and Economic Affairs, Ref:5007/7/3 dated 1994-09-30. VAT exemption will be according to the VAT Act. VAT zero rated under the 3rd schedule Section II 1998 Item 3 of the VAT Act cap 87.

In this section please provide information on items requested for Exemption of Duty and Taxes.

• Item: Name of item
• # of Packages: No of pallets or other large Packages
• Use: Top level use by categories listed. This identifies how product is used or purpose of product
• Type: Second level category, This identifies what type of product eg. Equipment, Drug
• Description: Third level category identifying more specific use or type of item
• # of Containers: Total number of salable or individual items in packages
• Size: The numerical weight or volume of container.
• Unt: Units of container’s content eg: gm, ml, kg, lb
• Value: Dollar value as appears on invoice.
